Here is a case i have seen:
A 41 yr old female who is a daily labourer (sweeper) came with c/o anuria since 1 day
She was apparently asymptotic 6 months back had bleeding per vaginal for which she consulted gynecologist and put on medication (no data available)
2 months back she had fever which is on and off associated with sob at rest (on and off) And also complaints of B/l pedal Edema (on and off) pitting type upto ankle
Then from 2 days she complaints of vomitings 2 episodes /day and palpitations
H/o headache (on and off) associated with neck pain n Nausea
No h/o burning mixture toon, pain abdomen, loose stools, chest pain, sweating, and giddiness.
Known case of hypertension since 1year. Used medication for 1 month and then stopped ( unknown)
3 years back nose surgery done ( ? DNS)
She has decreased appetite and decreased frequency of micturition
General examination
Patient is conscious and coherent
Pallor ++
No cyanosis, icterus, clubbing, lymphadenopathy
Vitals:
Temp 98.6F
Pulse 98bpm
Bp 180/120mmhg
RR 23cpm
SPO2 90% at RA
GRBS 111mg/dl
